1. Medical Care Model
Summit Performance & Wellness MGMT LLC is a management services organization (MSO) that provides non-clinical billing, technology, and patient support services. All medical evaluation, prescribing, and clinical oversight is provided exclusively by Wellness MD PC, a licensed Colorado medical professional corporation. No clinical decision is made by Summit or its staff.
2. Compounded Medications
Many Summit programs include compounded medications prepared at FDA-registered 503A or 503B pharmacies in the United States. Compounded products are not reviewed or approved by the FDA for safety, efficacy, or manufacturing in the same way as commercially manufactured drugs. Their use is based on a licensed provider's clinical judgment and your individual health profile.
- Medications are prescribed only after a licensed provider reviews your intake and health history.
- Dosing is individualized. Do not share medication or change your dose without provider approval.
- Store all medications according to the instructions provided by the pharmacy and your provider.
3. GLP-1 and Weight-Loss Therapy Risks
GLP-1 receptor agonists (such as semaglutide and tirzepatide) and related weight-loss therapies can cause significant side effects and are not appropriate for everyone. Common risks include:
- Nausea, vomiting, diarrhea, constipation, or abdominal discomfort
- Dehydration, gallbladder disease, or pancreatitis
- Low blood sugar, especially if combined with other glucose-lowering medications
- Delayed gastric emptying, which may affect absorption of oral medications
- Medullary thyroid carcinoma risk or MEN2 history is a contraindication
- Pregnancy, breastfeeding, or planned pregnancy are contraindications
You must disclose all medical conditions, current medications, supplements, and allergies during your intake. Tell your provider immediately if you experience severe abdominal pain, persistent vomiting, jaundice, or signs of an allergic reaction.
4. Peptide and Hormone Optimization Risks
Peptide therapies (such as BPC-157, TB-500, CJC-1295/Ipamorelin, NAD+, PT-141) and hormone optimization protocols may affect multiple body systems. Potential risks include but are not limited to:
- Injection-site reactions, bruising, or infection
- Fluid retention, joint pain, or changes in blood sugar or insulin sensitivity
- Changes in sleep, mood, appetite, or energy
- Effects on fertility, libido, or endocrine function
- Interactions with other medications or supplements
Long-term safety data for many peptides is limited or emerging. Your provider will discuss whether potential benefits outweigh risks for your specific situation.
5. Injection and Administration Safety
- Wash hands thoroughly before handling medication or supplies.
- Use a new sterile needle/syringe for every injection and dispose of sharps in an approved sharps container.
- Rotate injection sites to reduce tissue irritation and lipodystrophy.
- Do not use medication that is discolored, cloudy, or past its expiration date.
- Follow the exact administration route (subcutaneous, intramuscular, intranasal, oral) prescribed by your provider.
6. When to Seek Emergency Care
For any medical emergency, call 911 or go to the nearest emergency room immediately. Do not wait for a response from your telehealth provider. Seek emergency care for:
- Difficulty breathing, swelling of the face or throat, or severe allergic reaction
- Severe or persistent abdominal pain with vomiting
- Chest pain, shortness of breath, or signs of a heart attack or stroke
- Fainting, severe dizziness, or confusion
- Signs of a blood clot (leg swelling, pain, warmth, sudden chest pain)
7. Telehealth Limitations
Telehealth is convenient but is not a substitute for in-person care for every condition. By using this service, you acknowledge that:
- Your provider may not be able to perform a physical exam or obtain real-time diagnostic tests.
- You are responsible for providing complete and accurate medical information.
- If your provider determines you are not a candidate for telehealth treatment, you may be referred for in-person evaluation.
- You should maintain a relationship with a local primary care physician for routine and urgent in-person care.
8. Drug and Supplement Interactions
Peptides, GLP-1 medications, and hormone therapies may interact with other prescription drugs, over-the-counter medications, supplements, and recreational substances. Always disclose:
- All current prescription and non-prescription medications
- Vitamins, herbs, peptides, or performance supplements
- Recreational drug or alcohol use
- Known allergies to medications or ingredients
9. Pregnancy, Fertility, and Breastfeeding
Many of the medications offered through Summit are contraindicated in pregnancy, while trying to conceive, or while breastfeeding. If you become pregnant, plan to become pregnant, or begin breastfeeding during treatment, notify your provider immediately and stop medication as directed.
10. Follow-Up and Monitoring
- Complete all requested lab work before starting and at intervals requested by your provider.
- Attend scheduled follow-up appointments and respond to provider messages promptly.
- Report side effects, changes in health status, or new medications as soon as possible.
- Do not continue therapy indefinitely without periodic re-evaluation by a licensed provider.
11. Contraindications and Disqualifying Conditions
Certain medical conditions may disqualify you from specific therapies or require additional evaluation before starting. These include, but are not limited to:
- Active cancer, chemotherapy, or recent radiation therapy
- Uncontrolled thyroid disease, diabetes, or cardiovascular disease
- Active substance abuse or severe psychiatric instability
- History of pancreatitis, gallbladder disease, or certain endocrine tumors
- Current anticoagulant therapy or bleeding disorders (for injection protocols)
12. Informed Consent
Before beginning any treatment, you will be required to review and sign informed consent and telehealth disclosure documents specific to the therapy you are receiving. These documents supplement this safety sheet and outline the risks, benefits, alternatives, and your responsibilities as a patient.
